When driving in the Netherlands, it’s wise to be prepared for any situation on the road. Filling out your details

Before your holiday, fill in as many details as possible on the form and keep the pre-filled copy in your car. This preparation will save time and reduce stress if you need to complete the form immediately after an accident. In the event of a traffic accident, enter your information in one of the columns provided on the form. Be sure to sign the form at the bottom before submitting it to your insurance company.

When another party is involved

If another party is involved in the accident, they should fill in their information in the other column and sign the form as well. Each party can then send a copy to their respective insurance companies. It’s perfectly acceptable for you to send an English version of the completed form to your insurance company, while the other party submits, for example, a French version to theirs. As long as both forms contain the same information and are signed by both parties, they will be valid.
