Download the Official Report Forms
The European Accident Statement is the official document used across Europe to record traffic accidents. Below, you’ll find links to download the official European Accident Statement in PDF format and in multiple languages via Car Travel Docs.
Whether you’re planning a road trip across Europe or just want to be prepared, our site makes it easier to access the forms you need quickly. In addition to accident forms, we share helpful travel tips and resources, including guides for travelers with food allergies through My Meal Cards.

Essential items to keep in your car
Besides the European Accident Statement report form, having other items in your car, such as a warning triangle, first-aid kit, and even snow chains are crucial to ensure safety during emergencies and compliance with legal requirements. These items help mitigate risks and avoid fines while traveling across different European countries, where specific regulations may vary. For detailed information on required items by country, visit the Essential items you must have in your car webpage.
When language becomes a barrier
Imagine you’re on holiday and, unfortunately, you have a car accident with someone who doesn’t speak your language. Filling out the necessary accident report form can become a challenging task when communication breaks down. However, the process becomes much easier if you have the form available in their language as well. Simply hand them the foreign-language version of the form, and they can fill out their details immediately without confusion.
What’s more, it’s perfectly acceptable to send an English version of the completed form to your insurance company while they send a French version (or another language) to theirs. As long as the forms include the same information and are signed by both parties, everything will be in order. This approach ensures a smoother resolution for both sides.
