Download the Official Report Form
Scroll down until you see the buttons to open or download the official European Accident Statement in Portuguese (PDF).
The European Accident Statement is commonly used after a traffic incident to record details for insurance companies and authorities. We recommend downloading and printing the form in advance and keeping a copy in your car when traveling through Europe.

Essential items to keep in your car in Portugal
When driving in Portugal, it’s important to be aware of the legal requirements and to stay prepared for a range of road conditions. Portuguese law requires that certain items be kept in your vehicle at all times, including a reflective warning triangle and a reflective vest (which must be accessible from inside the vehicle). A flashlight and a first-aid kit are also recommended for added safety.
Carrying these essentials not only ensures compliance with local laws but also helps you stay ready for unexpected situations on the road. For detailed information on what’s legally required when driving in Portugal, visit the Essential items you must have for driving in Portugal webpage.
Filling out your details
Before your holiday, fill in as many details as possible on the form and keep the pre-filled copy in your car. This preparation will save time and reduce stress if you need to complete the form immediately after an accident. In the event of a traffic accident, enter your information in one of the columns provided on the form. Be sure to sign the form at the bottom before submitting it to your insurance company.
When another party is involved
If another party is involved in the accident, they should fill in their information in the other column and sign the form as well. Each party can then send a copy to their respective insurance companies. It’s perfectly acceptable for you to send an English version of the completed form to your insurance company, while the other party submits, for example, a French version to theirs. As long as both forms contain the same information and are signed by both parties, they will be valid.
