Download the Official Report Form
Scroll down until you see the buttons to open or download the official European Accident Statement in Polish (PDF).
The European Accident Statement is commonly used after a traffic incident to record details for insurance companies and authorities. We recommend downloading and printing the form in advance and keeping a copy in your car when traveling through Europe.

Essential items to keep in your car in Poland
When driving in Poland, it’s important to be aware of the legal requirements and to prepare for varying road conditions. Polish law mandates that certain items must be kept in your vehicle at all times, including a warning triangle and fire extinguisher. While not legally required, items such as a first-aid kit, and reflective vest are strongly recommended to enhance safety and be ready for emergencies. For requirements and recommendations, visit the Essential items you must have for driving in Poland webpage.
Filling out your details
Before your holiday, fill in as many details as possible on the form and keep the pre-filled copy in your car. This preparation will save time and reduce stress if you need to complete the form immediately after an accident. In the event of a traffic accident, enter your information in one of the columns provided on the form. Be sure to sign the form at the bottom before submitting it to your insurance company.
When another party is involved
If another party is involved in the accident, they should fill in their information in the other column and sign the form as well. Each party can then send a copy to their respective insurance companies. It’s perfectly acceptable for you to send an English version of the completed form to your insurance company, while the other party submits, for example, a French version to theirs. As long as both forms contain the same information and are signed by both parties, they will be valid.
