First Declaration page
Complete this page as soon as possible, sign it at the bottom, and then send it to your insurance company.

Second Accident Statement page
If another party is involved with the accident, you and the other party must enter the same details as in the first page. Both sign this page, and the other party sends it to their insurance company. You may also use a version of the page in a different language, if available.
